+ SUBSTITUTE SCHOOL NURSE (SCHOOL PROGRAM)

Substitute school nurse needed for school program Per Diem.  Must have School Nurse Certification or Substitute School Nurse Certification.

 

 

+ PROGRAM ASSISTANT (ADULT DAY HEALTH SERVICES PROGRAM

As part of a self directed team, the Program Assistant provides recreational and social activities to adults with intellectual challenges. In addition, the Program Assistant provides direct care in the area of Activities of Daily living to include, but not limited to mobility, self care, and feeding. The position includes driving on a daily basis.

A valid NJ Driver’s License and high school diploma or GED required.

Part-Time- Monday-Friday 10-4 with a 30 Minute break daily.

 

 

+  SOCIAL WORKER (ADULT DAY HEALTH SERVICES PROGRAM)

The Social Worker shall conduct assessments and integrate those findings into the Interdisciplinary Care Plan. The Social Worker shall conduct home visits, accessibility evaluations, and coordinate community programs or services. The Social Worker shall work with Nurse, Coordinator, Administrator and other interdisciplinary team members to ensure coordination and continuity of care using a team approach. Meet with all ADHS staff person(s) no less that twice monthly to address issues, provide client updates and to increase knowledge of inspecting agency requirements.

LSW or CSW.  1 year experience working with individuals with disabilities preferred

Part Time 10am-2pm Monday to Friday.

 

+  SKILL INSTRUCTOR (RESIDENTIAL PROGRAM)

1st Cerebral Palsy of NJ is seeking part time and per diem skill instructors for our residential program. Requirements: Must be 18 years of age or older. Must have a valid NJ drivers license. Must complete training classes as required by 1st Cerebral Palsy of NJ and DDD. Some Responsibilities include but are not limited to: working with adult consumers in a group home setting, assisting adults with daily care needs, enhancing consumers self-sufficiency, carry out in house and out of house group activities, accompany consumers to various medical appointments, and ensure the safety of consumers.
